The survey is confidential and those who fill out the questionnaire are not required to provide their names.

Those who do provide names and e-mail will be entered into a drawing for a $50 Barnes and Noble gift card that can be used at the University Bookstore for textbooks and safety supplies, or at any Barnes and Noble store.

Names will be separated from responses by the Office of Institutional Research and all responses will be considered anonymously. The gift card drawing will be conducted by IR on Dec. 1.

The University's Chemical Hygiene Committee is responsible for evaluating and administering the chemical safety aspects of all University programs involving the research and teaching use of hazardous chemicals. It facilitates the implementation of the Chemical Hygiene and Laboratory Safety Program through the Department of Environmental Health and Safety.

The goal of the Chemical Hygiene and Laboratory Safety Program is to minimize the risk of injury or illness by ensuring that researchers and students have the training, information, support and equipment needed to work safely in the laboratory.

It is a relatively new program that has seen significant developmental changes and improvements over the past few years. A key step in program development is an evaluation of the program's effectiveness and the survey is one piece of the program's evaluation plan
