Cost

The premiums for spousal coverage are age-graded, as age increases, the premium will increase.

The cost for this coverage will be deducted from your pay on an after-tax basis.

You are the beneficiary for any dependent coverage you choose.

You may elect coverage of $5,000 or $10,000 for dependent child(ren) up to age 26 without providing a Statement of Health (SOH).

You must complete a Statement of Health form if you increase coverage for your spouse OR if you choose life insurance for your spouse for the first time, other than as a new employee.

If you are a new employee, an SOH will also be required for a spouse/civil union spouse, if increasing or enrolling for coverage greater than $30,000.  If coverage is less than $30,000, no SOH is required as a new employee. 

If an employee and spouse both work at the University, dependent children are not permitted to be insured by both parents, and no one is permitted to be covered as both an employee and as a dependent.
