You are required to give a score for every subdomain.
You are not required to write a comment in every box but these comments help the student teacher improve.
Please complete the form within 48 hours of the formal observation.

Save the document as a pdf using the student's last name and the date of the observation in the following format: StudentLastName_MMDDYY (e.g. Kim_022412).

E-mail the document to the student teacher and the Social Studies Education Teaching Assistant, Melissa Maestri (maestri@udel.edu).

Attach the lesson plan, unit plan, and any lesson materials to the Observation Documents Coversheet.

You may:

• drop this off at the Social Studies Education office at 214 John Munroe Hall, or
• mail it to the Social Studies Education Office at University of Delaware, 214 John Munroe Hall, Newark, DE 19716, or
• give it to the student teacher to hand in.
