Ø    As a patient you have choices, rights and responsibilities.  We are concerned with providing you the very best in health care and ask that you participate in your own health care by being an active and informed consumer.

 

Ø    You Have The Right To:

Ø    Consideration

Ø    Dignity

Ø    Privacy

Ø    Confidentiality

Ø    An Explanation of Your Care

Ø    Know Availability of Services

Ø    Know All Fees For Service

Ø    Know Names and Positions of People Serving You

Ø    Review Your Medical Records and/or Receive a Copy

Ø    Request Corrections to Your Medical Record

Ø    Have Your Medical Records Released Only with Your Permission          

Ø    Request to Change Health Care Providers If Available

Ø    Accept or Decline Care/Treatment

 

Ø    You Have the Responsibility To:

 

Ø    Be Honest About Your Medical History - to include hospitalizations; surgery; medications including OTC products, dietary supplements; and any allergies or sensitivities                              

Ø    Report Any Changes in Your Health

Ø    Understand the Health Care and Services Offered

Ø    Follow Agreed Upon Health Care and Medical Instructions

Ø    Respect Other Students and Student Health Personnel

Ø    Make/Keep/Cancel Your Appointment

Ø    Provide Us with Feedback for Improving Services

 

Ø    Within the scope of the Student Health Service, the health care providers and staff are resources to help you achieve an optimal state of well-being
