UDSIS DATA Request Form

This form is available ONLY for UD Faculty and Staff to request UDSIS data in electronic format.  The data is provided for internal use at the University of Delaware. Data requests are generally completed and available five to ten business days from the date of the request.

Students:For transcripts, degree verification, etc. see the Forms area of UDSIS or refer to this menu.

Faculty / Staff: To request security access to the UDSIS Student Records system, use this form instead: UDSIS Student Records Access Request.
