You need Adobe Acrobat Reader for the following forms. If you don't have it download it here.

For registration: download the forms, print them out, fill them out, and then bring to the Intramural Office, (134 CSB) during the posted registration times.

Team Registration Form is only availabe at the CSB Cashier. Only a couple of things that need to be filled out.

Individual Registration Form is only available in the IM Office. Needs to be signed by the specific person.

Team Roster Form is only available in the IM Office. Due to security and eligibility issues, all team members will need to come to the office themselves to join a team.

Team Re-Registration (only for teams that are playing in League 1, registering for League 2 and have an active Bond on file [i.e. haven't forfeited])

Tournament Registration (This form must be turned in if your team wishes to play in the post-season)

Free Agent Registration, ask for the Free Agent Notebook in the office. Sign up and hopefully there is a team that needs players.

Officials Application Click on the link to the jobs page. to add yourself to the mailing list. You should come to training for the sport that you want to officiate. In general training is the week before a sport start registration. Take a look at the Sports Offered.
