- Student Health Service
- The Student Health Service is located in Laurel Hall, and is staffed by nurses 24 hours a day and 7 days a week (while school is in session). Physicians are on duty from 8:00 a.m. to 5:00 p.m., Monday - Friday. A Doctor is on call after hours. Phone: 831-2226.
- Medical Insurance
- All international students are required to have adequate medical insurance. The cost of the basic plan is automatically billed every semester to your student account, listed as MANDATORY MEDICAL INSURANCE. Coverage must be obtained for dependents at www.universityhealthplans.com. Students may apply for a waiver of the insurance if they are able to show proof of comparable medical insurance. Wavier forms must be completed NO LATER THAN THE LAST DAY OF FREE DROP ADD each semester and submitted with proof of insurance to 234 Hullihen Hall, Office of Graduate & Professional Education.

The enrollment period for 2008-2009 begins on August 18th and continues through September 15th. If a student enrolls in this time period, the coverage is valid beginning September 1st. To enroll in the insurance plan, please use and follow the directions on this form:

For International Students:
International students have been enrolled via their student bill and coverage begins on September 1st in the Blue Plan. However, if the international student wishes to enroll in the Gold plan or add dependents, please use and follow the directions on this form:
