Practicum Orientation Checklist
The student is responsible for returning a completed Clinical Practicum Orientation Checklist to the University Education Coordinator.

Student's Evaluation of Practicum
The student will be asked to evaluate his/her clinical experience in each discipline -- chemistry, hematology, microbiology, urinalysis, immunohematology. Student identification is not required on the Site Evaluation Forms. However students must have a means to notify the University education coordinator that they have completed the evaluation. The education coordinator or department chairperson will share the information in composite form with the affiliate and with the University faculty.

Urinalysis Tally Form
A minimum of 30 complete urinalyses should be performed (physical, chemical, microscopic). The emphasis should be placed on evaluating high quality urines to ensure that all major formed elements are observed. When a facility uses fully automated urinalysis, the student must still experience sufficient manual microscopics to observe all major formed elements. Student is responsible for returning the Urinalysis Tally to the Education Coordinator. Student is responsible for returning the Urinalysis Tally Form to the Education Coordinator.
