The formed element shown is a waxy cast. Waxy casts result from the coalescing of the protein in the matrix of the cast and further degeneration of cellular elements entrapped in the cast. Waxy casts are found in patients with long standing chronic renal disease or chronic renal failure. Their presence implies tubular obstruction with prolonged stasis, which correlates with the decreased urine output experienced by the patient.
