1)       What could the unknown moving element be (refer to picture and video)?  

The unknown element is trichomonas vaginalis (TV).  TV is one of the hardest formed elements to identify.  The four anterior flagella and the one posterior flagellum are exceedingly hard to see.  The motion of the parasite is the most reliable defining characteristic.
