Urine is sterile because it is an ultrafiltrate of the blood plasma.  The only way that bacteria can affect the urinary tract and hence enter the is due to fecal contamination which on 85% cases are gram negative rods which are normal flora in the digestive tract.  Such an infection is called endogeneous infections.  Infections can also be caused due to sexual transmission.
