Below is a complete list of Housing forms. Online forms will be electronically submitted to Housing Assignment Services or the Student Information System (UDSIS). PDF forms must be printed, and then mailed, faxed, or delivered to the Housing Assignment Services office (5 Courtney Street, Newark DE 19716). Please send an e-mail to
