On-Campus Housing Forms

Below is a complete list of Housing Web forms. Click the form that applies to a particular situation, complete and submit. The information collected from the forms below are electronically submitted to Housing Assignment Services or the Student Information System (UDSIS). Please send e-mail to

• Semester Room Change (available from Mid September-December 19, 2009 to change your Fall 2009 room assignment))
• Request for Release (complete form and return to Housing Assignmment Services with appropriate documentation)
• Summer Room Change Request (form currently not available, please send an email to ud-housing@udel.edu
• Roommate Verification Form (complete this form to verifiy another student as your roommate)
• Cancellation Form for Fall 2008 On-Campus Housing (form currently not available, please send an email to ud-housing@udel.edu)
• Request for Release for Medical, Psychological, or Disability Needs (complete form if you are requesting release due to a medical situation)
• Financial Hardship (print both pages, complete and fax or mail to Housing Assignment Services)
• Mid-Year Room Change (this form is currently not available)
• Deferral Form
• New Student Housing Application (deadline to apply is May 1. Changes to Housing Information accepted until May 31)
• Returning Student Housing Application (available from February 26-March 15, 2008)
• Greek House Cancellation Form (form not available)
• Returning Student UHO Supplemental Form (this form is no longer available)
• New Student UHO Supplemental Form (this form will be available from March 15-May 1, 2008)
• Medical, Psychological, or Disability Needs Form (complete, print, and mail by June 13, 2008)
• Summer Session Housing Application (complete and submit for all Summer Sessions only)
• Summer Session Housing Cancellation Form (print, complete and mail to Housing Assignment Services)
• Winter Activity (form not available)
• Housing Request for Continuing Education Students (complete, print, and mail by June 17, 2008)
• Early Check-in Sign-Up Form (this form will be available until August 22, 2008)
