      d. Problems in implementation

   

      e. Recommended modifications if program is to be repeated

   

      f. Participant program evaluations

   

3. Submit to the graduate course instructor within one week following presentation:

   

      a. Undergraduate student’s evaluation of clinical experience

1. Complete evaluation of clinical experience and return original to graduate student and a photocopy to course instructor

   

2. Communicate completion of health promotion project to course instructor
