Intention Forms – Guidelines
Halfway through each session, Intention Forms are distributed to all ELI students. This is your opportunity to tell the ELI what you plan to do next session — do you plan to stay at the ELI for another session, or do you plan to leave? This page is meant to help you to complete your Intention Form.
Section 1
This section contains the student’s vital information (name, student ID number, visa type, etc.). Verify that all this information is correct!
Section 2
This part of the intention form tells the ELI whether you plan to stay for another session or if you plan to leave at the end of this session. Remember, this page is like a contract. We use this information to manage your visa status (i.e., extend your I-20), to prepare your bill for next session, and to help you to make your academic plans.

If you plan to stay at the ELI for another session:
If you will stay at the ELI, that is great news! Follow these steps:
- Check the box on the form that says that you plan to stay at the ELI.
- Check the boxes of ALL the ELI sessions that you wish to attend. NOTE: If your I-20 is going to expire and you are not sure if you want to attend MORE than one session, check the box for next session only.
- If your I-20 is going to expire, please view “How to extend my I-20.”
- Tell us how we should make your bill. Would you like your next bill to show the price for only one session, or would you like to be billed for two sessions (to have the tuition discount)?
- Sign the intention form.
- Turn over the form and complete Part 3 – Choose your classes. If you have a question about which class to choose, contact Deb Detzel .
If you plan to leave the ELI at the end of this session:
If you will leave the ELI after this session, we are sorry to see you go! Follow these steps to complete your intention form:
- At the bottom of page 1 of the Intention Form, check the box that says that you will leave the ELI.
- Sign the form.
If you plan to leave the ELI earlier than the end date on your I-20 form, you must make an appointment for an exit interview with Dr. Stevens. To make an appointment, please send an e-mail to the ELI Receptionist.
If you do not know if you will stay at the ELI
If you are not sure whether you will stay at the ELI, make an appointment to see a counselor in the ELI Admissions Office before the deadline printed on the Intention Form. It is important that the counselors know what your situation is so they can help you.
If you are waiting for the results of your scholarship application, you must still submit your intention form with a bank statement to the ELI Admissions Office. Write on the top of your intention form that you are waiting for the results of the scholarship application. If you are not awarded a scholarship and you plan to stay at the ELI, you must submit a bank statement that shows that you have enough funds to afford another session ($4,650).
