Submission Form for DNA sequencing
All samples must be submitted with a completed submission form. Print the form and fill out completely. Processing may be delayed if samples are submitted with an incomplete form. The PDF version must be filled out by hand, the Word version allows data entry using MS Word - this is especially helpful for entering common data (name, PI name, phone #, etc.)
