COMPLETING THE FINAL EVALUATION REPORT

The final evaluation report represents the student to a future employer. Care should be taken to make sure that these reports are completed nearly and with correct grammar and spelling.

ORIGINAL copies of the final evaluation reports should be sent to the Office of Clinical Studies. Students can go to the OCS and sigh the release form for their reports to be sent to Career Services. PLEASE DO NOT GIVE THE ORIGINAL COPY FO THE REPORT TO THE STUDENT. When the OCS receives a copy and not the original, employers are being sent a copy of a copy of a copy that is usually not a very good copy.

Please be sure to complete the evaluation form in its entirety. When the OCS completes certification forms, the number of hours, the teaching field, grade level(s), etc. must be documented. If this information is missing from the report, it is very difficult to complete the certification forms accurately. When writing reports of special education placements, it is extremely important to describe how the pupils in the class were classified or identified. Certification forms require this information.

Remind students to record their hours daily. Students completing one experience for nine credits need a total of 360 hours. Students completing multiple experiences for 6 credits each need a total of 240 hours for each experience.

Please encourage cooperating teachers to record the accurate number of observation hours and teaching hours. The total number of hours should not simply be split in half (e.g., 180/180 or 120/120). Recording hours in this manner does a disservice to the student teacher since most of our student teachers attain far more teaching hours than observation hours.

Cooperating teacher should be reminded that they should not use final evaluation forms from handbooks or forms that are three-holed punched. It is very important that the student's ID number is recorded on the form, as the OCS and Career Services need this number in order to scan the form into the system.

Blank copies of the final evaluation form are available on the disks in the manuals that are distributed by you to the cooperating teachers.

  • Disks are to be kept in a secure place since we do not want students writing their own final evaluation reports.
  • When completing the mid-placement evaluation, please be sure to write "mid-placement evaluation" prominently on the report. Some students have preferred their mid-placement evaluation to their final evaluation and have tried to put the mid-placements evaluation in their credential file.
  • All disks are to be collected at the end of the experience and returned to the coordinator, along with the manual, the blue Praxis booklet and the Student Teaching Guidelines. The forms are consumable, but we cannot afford to replace the disks, Praxis booklets, and Guidelines each semester/each year. These items are to be kept by you or returned to the Office of Clinical Studies to be passed on to the next group of cooperating teachers. If we do not receive them from you, we will assume that you are taking responsibility for distributing them to the next group of cooperating teachers.
  • Both a MAC and a PC version of the disks are included in the manual. Please be sure to collect both disks at the end of the placement.
  • Remind cooperating teachers that they can keep a copy of their completed report by saving it to their hard drive or copying it to another disk. Since the disk that they have been given will be passed on to another teacher, the student teacher's report should not be saved onto that disk.
  • Teachers and supervisors should print a hard copy of the report and send it to the OCS. If you would like to collect all of your reports from your cooperating teachers, that is fine, however, please be certain to deliver the reports to the OCS.
