    1. you have received instruction in its use from Kirk Czymmek,
    2. you have received radiation safety training from the UD Radiation Safety Officer (x3450), and
    3. you have been issued a radiation monitoring badge.

  2. Do not operate this x-ray device if the XRAY ON lights do not operate or there is any other equipment malfunction.  If you experience equipment problems, post the equipment “Out of Service” and contact Kirk Czymmek (x3450).

  3. If you believe you have been over-exposed to radiation, or for any other radiation safety concern, contact the Kirk Czymmek (x3450).

  4. Keep the door to the room locked at all times.

  5. Wear your radiation monitoring badge at all times when operating this x-ray device.  Do not wear a badge assigned to another person.  Store your badge in the assigned location in room 006 Wolf. DO NOT store your badge in the x-ray room!

  6. A low-level radiation field exists near the device when the x-rays are ON.  Although working in a radiation field of this magnitude (even for prolonged periods) will not significantly increase your risk to a detrimental health effect, it is advisable, whenever possible, to minimize your radiation exposure.  For this reason you should limit time spent in close proximity to the scanner when x-rays are ON (note area marked on floor)
