Delaware Transition Initiative: The Alfred I. duPont Hospital for Children Transition Survey Project


The Delaware Transition Initiative: The Alfred I. duPont Hospital for Children Transition Survey Project explored the medical transition experiences of young adults and their families in the process of transitioning from the pediatric health care system to the adult health care system.

The Center for Disabilities Studies (CDS) was contracted by the Office of Children with Special Health Care Needs, Division of Public Health (DPH), Delaware Health and Social Services (DHSS), to carry out a study on the experiences of young adults with the medical transition process from pediatric services (Alfred I. duPont Hospital for Children) to adult services. The study was conducted between January 2006 and May 2007.

This project was supported by the Champions for Progress Institute, Utah State University, and the Delaware Coordinating Council for Children with Disabilities (DCCCD).
