INSTRUCTIONS

• Read the instructions twice
• Select the group letter of the group you belong to
• Read the previous instruction once more
• Enter your first name followed by your lastname and include a blank in between names
• Click OK
• Enter your Social Security number for verification
• Enter grades in each box for each group from 0 to 10 using whole numbers
• Submit this form by pressing on the Submit this Form to Alex button at the very bottom of the page
  If you made a mistake on the form you will be notified and you will not be able to submit until you correct your mistake
• E-mail grading rational to Alex
