Health survey sent to employees
University employees recently received a risk assessment survey
from the state of Delaware similar to one distributed by the UD about
three years ago and used to plan the campus Wellness program. The
comprehensive Health Risk Assessment (HRA) is being provided at no
cost to employees and spouses acovered under the state's health care
group.
The survey is designed to help participants detect or monitor
health risks and provide assistance in maintaining a healthy
lifestyle. By completing the health-risk assessment questionnaire and
participating in on-site screenings, individuals will become more
aware of the lifestyle risk factors and behaviors that affect their
health and well-being.
Sections in the questionnaire request information on personal
history, exercise level, eating habits, alcohol/drug use,
stress/coping techniques, safety, medical care and exercise
preference.
The HRA is administered by the Delaware Center for Wellness,
which will provide participants with a confidential report.
Participants also will be able to schedule a follow-up screening for
such risk factors as cholesterol and blood pressure levels.
The state will receive only a summary report that will identify
risk factors affecting a broad population of state employees. This
information will be used to plan health promotion programs to reduce
identified health risks for employees throughout the state.
The state's packet includes a letter of explanation, list of
screening sites (including address, date, time and phone number), a
Personal Wellness Profile questionnaire and a postage-paid return
envelope.
