Attention Blue Cross Blue Shield Subscribers

Reminder: Effective July 1, 1996, the sole provider for your
laboratory services is LabCorp. All Blue Cross and Blue Shield
subscribers with Basic, Comprehensive, Staff Model HMO and Independent
Practice Association (Total Health Plus) coverages must use a LabCorp
facility in order to receive full benefits.
