     The University pays the full cost of Basic coverage for eligible
employees* and any dependents who are enrolled in the group program.
Employees can enroll in optional coverage-such as Basic Plus,
Comprehensive, HMO, Total Health Plus and Principal Health Care- providing
they pay the additional cost of the option selected. The amounts shown
above in the "Employee Payment" column are the differences between the cost
of Basic coverage and the total monthly premium of the optional coverage.

* Employees who are employed less than 75 percent time receive a University
  payment proportionate to the percent of time worked.
