Flexible Benefits '94-'95
Keep your Flexible Benefits Book

     This year your personalized enrollment form and your Flexible Benefits
book will be mailed together, instead of sent to you separately.
     Your enrollment form will be placed inside the front cover of the
Flexible Benefits book.
     Information about the 1994-95 open enrollment period, including the
workshop schedule, is printed on the inside cover of your book.
     When you receive your Flexible Benefits package, please take some time
to review the information.
     If you decide to change any of your benefits selections or participate
in a Flexible Spending Account, you must complete the enrollment form and
return it to the Benefits Office by May 13.
