Flexible Benefits '94-'95
How to change your flexible benefits in six easy steps

     If you wish to make changes to your flexible benefits program, just
follow these easy steps:
     * Make every effort to fully understand your entitlements. Read as
much as you can about the program and pay particular attention to the
flexible benefits book. Detailed health plan booklets are also available in
the Benefits Office. Try to attend a workshop in early May. If you cannot
attend, call the Benefits Office for information.
     * Evaluate your present choices. Review your needs for the coming year
and compare coverage with cost.
     * List your new choices on the personalized enrollment form which you
will receive in early May. Check all boxes, sign and return to the Benefits
Office. If you do not change coverage, you need not return the enrollment
form. However, all FSA participants must return a completed form (see final
step).
     * Changing medical plan or coverage requires that you complete an
application and submit it with your enrollment form.
     * Enrolling in dependent life insurance the first time or applying for
more than $400,000 of employee life insurance at any time requires approval
from the carrier. Request an evidence of insurability form from the
Benefits Office, complete and return with your enrollment materials.
     * BE SURE TO RETURN YOUR FLEX ENROLLMENT FORM IF YOU WANT TO
CONTRIBUTE TO A FLEXIBLE SPENDING ACCOUNT (FSA).  THIS IS A FEDERAL
REQUIREMENT.
     THE DEADLINE FOR RETURNING ENROLLMENT FORMS IS MAY 13.
