How to change flexible benefits in six steps

Just follow these easy steps to make changes to your Flexible Benefits
Program.

       * One-Make every effort to understand your entitlements. Read as
much as you can about the program and pay particular attention to
the flexible benefits book and the individualized health care
plan booklets. Try to attend one of the workshops in the Perkins
Student Center during early May. If you cannot attend, call the
         Benefits Office for assistance.
       * Two-Evaluate your present selections by comparing your needs for
the coming year with options, provisions and costs.
       * Three-Put your new choices on the personalized enrollment form
which will be mailed to you in late April. Check all boxes, sign,
and return it to the Benefits Office. Note: If you are not
changing coverage, you need not return the enrollment form.
However, all FSA participants must return a completed form (see
         step six).
       * Four-Changing medical coverage requires a completed application
which must be submitted with your enrollment form.
       * Five-Enrolling in dependent life insurance the first time or
applying for more than $400,000 of employee life insurance at any
time requires approval from the carrier. Request an evidence of
insurability form from the Benefits Office, complete and return
         it with your enrollment materials.
       * Six-Be sure to return your flex enrollment form if you want to
contribute to a flexible spending account (FSA). This is a
         federal law requirement.

The deadline for returning enrollment forms is May 14.
