Changes in employee health and prescription drug plan in effect
 
10 a.m., July 3, 2003--The Office of Human Resources reminds eligible UD employees that the benefit plan changes summarized below went into effect July 1, 2003.

Medical Insurance
As announced during the recent benefits open enrollment, new co-payments have been added and some copayment amounts have increased.

Comprehensive PPO

  • Sick visit - $15 copay
  • Specialist - $25 copay
  • Allergy testing - $25 copay
  • Allergy treatment - $5 copay

BlueCARE and Coventry HMOs

  • Hospital admission - $75/day, with $150 maximum copay per admission
  • Emergency room visit - $75 copay
  • Urgent care visit - $35 copay
  • Ambulance - $50 copay
  • X-ray - $15 copay per visit
  • CT scan, PET scan and MRI - $25 copay per visit
  • Outpatient surgery: doctor’s office - $20 copay; ambulatory center - $30 copay; hospital operating room - $75 copay

    Blue Cross Blue Shield of Delaware is currently printing new plan booklets that should be distributed to participants by the end of the month.

    Plan participants (new as well as current members) will receive new ID cards.

    Prescription Drug Program
    Step Therapy prescription protocols have been adopted when medications are prescribed for the following conditions: ulcer therapy, pain and inflammation, high blood pressure, rheumatoid arthritis and anti-depressants. This program is for NEW prescriptions written on or after July 1, 2003.

    Coordination of Benefits for Prescriptions: If your spouse or other dependent(s) has other health coverage that is primary (pays first), the prescription coverage through the state plan for those dependents will become secondary.

    Requirements for Supporting Documentation: The State Personnel Office (SPO) has announced new requirements effective July 1, 2003, for supporting documentation related to benefits enrollment. This year and every annual open enrollment thereafter, a completed Spousal Coordination of Benefits Policy form is required of individuals who cover a spouse under the health plan. The state coordinates benefits for an enrolled spouse who has other insurance or is eligible for other insurance.     Proof of eligibility is now required when enrolling a dependent for the first time. A copy of your marriage certificate is required if you are enrolling your spouse for the first time. A copy of the birth certificate is required if you are enrolling your dependent child for the first time.
