Standard Operating Procedures/Job Hazard Analysis:  Laboratory Operations

The Chemical Hygiene Committee must approve the use and purchase of highly toxic or carcinogenic material. The definitions of these materials are as follows, but consult Chapter 12 of the Chemical Hygiene Plan for further information:

  • A chemical with an LD50 of less than 50 milligrams per kilogram
  • Regulated by OHSA and listed by National Toxicology Program (NTP) and International Agency for Research on Cancer Monographs (IARC) as a carcinogen:
  • Overall Evaluations of Carcinogenicity to Humans
  • National Toxicology Program - Report on Carcinogens
  • Examples include, but are not limited to, Arcolein, Allyl Alcohol, Chlorine, Nickel Carbonyl, Benzene, Beryllium, Hexavalent Chromium Compounds and Ethylene Oxide. The SOP form also serves as guidance to complete this approval process.

  • Standard operating Procedure/Approval Form for Carcinogens and Highly Toxic Materials (Word Format)
  • Standard Operating Procedure/Approval Renewal Form For Carcinogens and Highly Toxic Materials (Word Format)
