Listed below are various forms used by the Career Services Center. This site was created to enable students, faculty, and employers easy access to these forms.

  • Workshop/Presentation Request Form Career Services Center staff are available to make presentations to campus groups (including classes and student organizations) on a wide variety of career planning and job hunting topics including: resume writing, interviewing, job search strategies, and CSC tours. You may request a presentation by completing this interactive form.


  • City of Newark/University of Delaware Nomination Form for Volunteer Service Recognition: Each year The University of Delaware and the City of Newark recognize outstanding student volunteers and outstanding Student volunteer organizations. We need your recommendations to determine which students and groups are making a difference to our community. (Self nominations are acceptable.) Please go to the Volunteer Nomination Form and return it by April 8, 2011. For further information, contact Marianne Green at megreen@udel.edu or call 831-1231. (Self nominations are acceptable.)


  • I GOT A JOB! - Confidential Career Plan Survey You can inform the Bank of America Career Services Center of your job offer by clicking and completing a "Career Plan Survey Form" - All information is confidental and used only in reports such as "Average Salary Offered by Major" and no students' names are attached to the reports).
    • When you report your job acceptance to the Bank of America Career Services Center you also need to cancel any remaining interviews you had signed up for through the Campus Interview Program (Blue Hen Careers).
    • Remove your resume from any online resume books you had posted it in (including the resume books in Blue Hen Careers).


  • I'M GOING TO GRAD SCHOOL! You can inform the Bank of America Career Services Center of your acceptance to attend grad school by clicking and completing a "Career Plan Survey Form" - All information is confidental and used only in reports such as where UD grads continue their education and no students' names are attached to the reports).
    • When you report your graduate school acceptance to the Bank of America Career Services Center you also need to cancel any remaining interviews you had signed up for through the Campus Interview Program (Blue Hen Careers).

    • Remove your resume from any online resume books you had posted it in (including the resume books in Blue Hen Careers).

  • Career Assessment Request Form: Alumni interested in receiveing career assessments must fill out this form. Information is subject to verfication prior to assessment.
