Before you decide, note...
- This cost will be deducted from your paycheck on an after-tax basis.
- You are the beneficiary for any dependent coverage you choose.
- You must complete a Statement of Health form if you increase coverage two
steps OR if you choose dependent life insurance for the first time, other
than as a new employee.
- The age limit for children is 21 (and age 24 for full-time students)
